As a Nurse Practitioner in this flexible per diem role, you will provide essential home health risk assessments for Medicare Advantage plan members across Buncombe County, NC. Your clinical expertise will support early identification of health risks, delivering valuable care to individuals in their homes. Locations include Asheville and areas such as Montreat, Biltmore, and Woodfin, with each visit averaging about 45 minutes. Assessment types may vary from annual health evaluations to post-discharge visits based on member needs.
Schedule:
- Choose your own shifts between Monday and Sunday
- Design your own schedule to fit your personal and professional commitments
- Minimal weekly or bi-weekly hourly requirements based on your availability
Primary Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review and document previous and current medical histories, including all medical conditions and medication usage
- Offer education on health conditions and prescribed medications to promote understanding and compliance
- Identify care gaps and recommend solutions to improve patient outcomes
- No responsibility for treatment planning or medication prescribing
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Current BLS CPR certification
- MSN degree from an accredited nursing program
- Minimum of six months NP practice is preferred
