Deliver crucial care in the comfort of patients homes by performing comprehensive risk assessments for Medicare Advantage plan members across Buncombe County. This per diem nurse practitioner opportunity enables you to provide valuable insight and health education to seniors and recently discharged patients in a variety of home settings, including locations such as Montreat, Biltmore, Woodfin, and Asheville.

Schedule:

• Flexible shifts between Monday and Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Conduct annual and post-discharge home health risk assessments, focusing on thorough evaluation of prior and current medical history, conditions, and medication usage.
• Assess each patients health needs in a 45-minute visit, documenting findings and identifying care gaps.
• Educate patients about their medical conditions and prescribe actionable steps to support better outcomes.
• Recommend interventions or resources to address any care gaps you find, ensuring comprehensive, patient-centered evaluations.
• No prescribing of medications or treatment plan development required.

Qualifications:

• Active Nurse Practitioner license in NC
• Board certification through AANP or ANCC
• Current BLS/CPR certification
• MSN (Master of Science in Nursing) from an accredited institution
• Minimum six months of recent nurse practitioner experience preferred

This role offers compensation between $80 and $90 per completed evaluation, along with a mileage stipend and payment for administrative work. Candidates will have the flexibility to build a schedule that fits their lifestyle while making a direct impact in the community through high-quality, independent patient assessments.
