Opportunity available for a Nurse Practitioner with home health experience to conduct risk assessments. This per diem position allows you to independently create your own schedule, with flexible shifts offered seven days a week. Minimum weekly or bi-weekly hourly commitments ensure adaptability to your lifestyle and availability.

Schedule:

• Flexible shifts Monday through Sunday
• Self-selected scheduling
• Commit to a minimum of weekly or bi-weekly hours

Key Responsibilities:

• Perform thorough annual risk assessments for Medicare Advantage members during home health visits
• Review patients prior and current medical history, assess current medical conditions and medication use
• Educate patients on their conditions, available treatments, and proper medication management
• Identify gaps in care and recommend actionable solutions
• Documentation and assessment only; no prescription writing or treatment plan creation required

Qualifications:

• Active Nurse Practitioner license in Virginia
• Board Certification through AANP or ANCC
• Current Basic Life Support (CPR) certification
• MSN degree from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred

Compensation for this role includes $80$90 per completed evaluation, with a mileage stipend and administrative time included.
