Make an impact in your community by providing essential home health assessments as a licensed Nurse Practitioner. This per diem opportunity offers flexible schedulingchoose your hours between Monday and Sunday and determine your own level of weekly or bi-weekly commitment.

Responsibilities:

• Conduct home health risk assessments for Medicare Advantage plan members in Amherst County, including Amherst and Madison Heights.
• Complete comprehensive evaluations of patients current and prior medical histories, conditions, and medication regimens.
• Educate patients on their diagnoses and prescribed treatments, helping empower them to better manage their health.
• Identify gaps in care, document findings, and provide clear recommendations for improving health outcomes.
• No responsibilities for developing treatment plans or prescribing medications.

Schedule:

• Flexible shifts available Monday through Sunday
• Build your own schedule based on availability and preference
• Opportunities to accept minimum weekly or bi-weekly hourly commitments

Preferred Experience & Qualifications:

• Active Nurse Practitioner License in Virginia
• Board certification through the AANP or ANCC
• Current BLS/CPR certification
• MSN from an accredited school of nursing
• At least six months of recent NP experience is highly desired

Enjoy per evaluation pay rates of $80-$90, along with a mileage stipend and compensation for administrative time.
