Job Description

Deliver personalized care as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, youll visit Medicare Advantage plan members in a variety of neighborhoods throughout Seminole County. Assessments typically last around 45 minutes, and may range from annual health reviews to post-discharge follow-up visits. Locations during your shifts could include Sanford, Altamonte Springs, Casselberry, and other communities within the county.

Schedule:

  • Flexible work shifts between Monday and Sunday
  • Option to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive annual risk assessments for Medicare Advantage health plan members in their homes
  • Review and document prior/current medical history, active conditions, and medication regimens
  • Identify gaps in care and recommend solutions for improved health outcomes
  • Provide patient education about medical conditions and medication management
  • No requirement to develop treatment plans or prescribe medications

Required Qualifications:

  • Active Nurse Practitioner license in Florida
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN from an accredited nursing program
  • Minimum 6 months of recent experience as a Nurse Practitioner preferred
