Step into a dynamic per diem opportunity delivering essential home health risk assessments as a Nurse Practitioner in Lake County, FL. This role focuses on evaluating Medicare Advantage plan members through scheduled visits in Clermont, Leesburg, Eustis, and surrounding areas. Each home health visit averages 45 minutes and may involve annual health assessments or post-discharge evaluations. Schedule
- Flexible shifts available seven days a week
- Self-scheduling: tailor shifts to your availability

Key Responsibilities
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review and document previous and current medical histories, clinical conditions, and medication usage
- Educate patients on chronic conditions, medications, and prevention strategies
- Identify care gaps and offer recommendations to improve outcomes
- No treatment planning or medication prescription required
Qualifications
- Active Nurse Practitioner license in Florida
- Board Certification (AANP or ANCC)
- MSN degree from an accredited nursing program
- Current BLS/CPR certification
- Minimum six months of recent Nurse Practitioner experience preferred
