Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule and manage your time
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage plan members in their residences
• Review and document prior/current medical history, ongoing health conditions, and medication regimens
• Educate members about their diagnoses, treatment, and proper medication use
• Identify care gaps and recommend appropriate measures for improved outcomes
• No need to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner licensure in North Carolina
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• Graduation from an accredited MSN program
• At least 6 months of recent Nurse Practitioner experience (preferred)
