Job Description

Advance your career in a dynamic per diem role conducting home health risk assessments for Medicare Advantage plan members throughout Wake County, NCincluding the vibrant communities of Raleigh, Cary, and Apex. As a Nurse Practitioner with a focus on home health, youll play a critical part in ensuring older adults receive the care and education they need to thrive at home.

Schedule:

  • Flexible shifts available seven days a week
  • Design your own work schedule
  • Choose your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage plan members
  • Review and document patients prior/current medical history, conditions, and medication usage
  • Educate patients about their medical conditions and prescribed medications
  • Identify care gaps, offer actionable recommendations, and support improved health outcomes
  • Role is focused strictly on assessmentsthere is no treatment planning or medication prescribing required
  • Each visit typically takes about 45 minutes, with opportunities for both annual wellness and post-discharge assessments

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • Board certified by AANP or ANCC
  • At least 6 months of recent Nurse Practitioner experience preferred
  • Masters degree in Nursing (MSN) from an accredited nursing school
