Deliver essential healthcare directly to patients with this rewarding per diem opportunity. As an experienced Nurse Practitioner, you will conduct home health risk assessments for Medicare Advantage plan members across Wake County, including Raleigh, Cary, and Apex. Home visits range from annual health assessments to post-discharge checkups, typically lasting around 45 minutes. The role offers the flexibility to create your own schedule, with shifts available any day of the week.

Key Responsibilities
- Perform comprehensive home health risk assessments for a Medicare Advantage health plan
- Review and evaluate patients medical history, current health status, and medication usage
- Educate patients on their medical conditions and medication plans
- Identify care gaps and recommend solutions to optimize health outcomes
- No requirement for treatment plan creation or prescribing medications
Qualifications
- Active Nurse Practitioner license in North Carolina
- MSN degree from an accredited nursing school
- Current BLS (CPR) certification
- Board Certification (AANP or ANCC)
- At least 6 months of nurse practitioner experience preferred
