Advance your career in a dynamic per diem role conducting home health risk assessments for Medicare Advantage plan members throughout Wake County, NCincluding the vibrant communities of Raleigh, Cary, and Apex. As a Nurse Practitioner with a focus on home health, youll play a critical part in ensuring older adults receive the care and education they need to thrive at home.
Schedule:
- Flexible shifts available seven days a week
- Design your own work schedule
- Choose your minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Review and document patients prior/current medical history, conditions, and medication usage
- Educate patients about their medical conditions and prescribed medications
- Identify care gaps, offer actionable recommendations, and support improved health outcomes
- Role is focused strictly on assessmentsthere is no treatment planning or medication prescribing required
- Each visit typically takes about 45 minutes, with opportunities for both annual wellness and post-discharge assessments
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certified by AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- Masters degree in Nursing (MSN) from an accredited nursing school
