Schedule:
- Flexible shifts available Monday through Sunday
- Set your own work schedule
- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
- Review each patients prior and current medical history, conditions, and medication list
- Deliver clear education on health conditions and medication management
- Identify care gaps and make recommendations for improved outcomes
- Complete thorough clinical documentation; treatment planning and prescription services are not required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification via AANP or ANCC
- Current CPR certification (BLS)
- MSN from an accredited nursing institution
- At least 6 months of recent NP experience preferred
This per diem role offers a pay rate of $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
