Make a meaningful impact by providing crucial healthcare support through home health risk assessments in Johnston County and the surrounding areas. This per diem opportunity is ideal for nurse practitioners who appreciate flexibility and autonomy. Youll perform visits in Cleveland, Clayton, Smithfield, Selma, and additional locations as assigned, all while enjoying a schedule that fits your lifestyle.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: choose the days and hours that work for you
- Minimal weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive in-home health risk assessments for Medicare Advantage plan members
- Complete evaluations focused on prior/current medical history, health conditions, and medication use
- Offer patient education regarding health conditions and medication management
- Identify gaps in patient care and recommend actionable solutions
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification (AANP or ANCC)
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- Minimum of 6 months nurse practitioner experience preferred
