Bring your advanced clinical skills to a dynamic per diem opportunity focused on improving community health. In this home health nurse practitioner role, you will conduct scheduled risk assessments for Medicare Advantage plan members throughout Tioga County, NY, including locations such as Waverly, Owego, and Newark Valley. Each visit typically lasts around 45 minutes and may consist of annual assessments or post-discharge evaluations.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own personalized schedule
  • Commit to either weekly or bi-weekly hourly minimums

Key Responsibilities:

  • Perform thorough annual or post-discharge home health risk assessments
  • Review and evaluate prior and current medical history, ongoing conditions, and medication records
  • Educate patients on medication regimens and chronic conditions
  • Identify gaps in care and make recommendations to encourage best health outcomes
  • Administrative tasks directly related to visits
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Board certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN degree earned from an accredited institution
  • At least 6 months of current nurse practitioner experience preferred
