Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own work schedule
- Choose a weekly or bi-weekly hours commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members in Williamson County, including Franklin, Brentwood, and Spring Hill
- Evaluate patients prior and current medical history, assess existing conditions, and review medication regimens
- Offer patient education on health conditions, medications, and overall wellness
- Identify any gaps in care and make recommendations to enhance patient outcomes
- Visits may include annual assessments or post-discharge follow-ups
- No requirements for creating treatment plans or prescribing medications
What Were Looking For:
- Current Nurse Practitioner license in Tennessee
- Active board certification (AANP or ANCC)
- CPR certification (BLS)
- Masters degree (MSN) from an accredited nursing program
- At least six months of recent experience as a Nurse Practitioner is preferred
- Strong assessment, communication, and patient education skills
- Comfort and reliability working independently in a home health setting
