Schedule:

  • Flexible shifts available between Monday and Sunday

  • Choose a weekly or bi-weekly hours commitment

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members in Williamson County, including Franklin, Brentwood, and Spring Hill
  • Evaluate patients prior and current medical history, assess existing conditions, and review medication regimens
  • Offer patient education on health conditions, medications, and overall wellness
  • Identify any gaps in care and make recommendations to enhance patient outcomes
  • Visits may include annual assessments or post-discharge follow-ups
  • No requirements for creating treatment plans or prescribing medications

What Were Looking For:

  • Current Nurse Practitioner license in Tennessee
  • Active board certification (AANP or ANCC)
  • CPR certification (BLS)
  • Masters degree (MSN) from an accredited nursing program
  • At least six months of recent experience as a Nurse Practitioner is preferred
  • Strong assessment, communication, and patient education skills
  • Comfort and reliability working independently in a home health setting
