Deliver high-quality patient care in a dynamic per diem opportunity as a Nurse Practitioner specializing in home health services. This rewarding position involves conducting home health risk assessments for Medicare Advantage plan participants across Polk County, including Urbandale, Johnston, and Clive. Visits focus on either annual health assessments or post-discharge evaluations, each typically lasting 45 minutes. As part of this vital outreach, youll play an essential role in supporting patient wellness in their own environment while enjoying the flexibility to design your own work schedule.

Schedule:

• Flexible shifts available seven days a week
• Self-scheduling to fit your lifestyle
• Minimum weekly or bi-weekly hourly commitment, based on availability

Essential Functions:

• Complete comprehensive home health risk assessments for Medicare Advantage members
• Review and document prior and current medical history, health conditions, and medication use
• Provide clear education to members regarding their medical conditions and prescribed medications
• Identify care gaps and supply actionable recommendations to ensure continuity of care
• No treatment plan creation or prescription writing required

Preferred Qualifications:

• Active IA Nurse Practitioner license
• Current Basic Life Support (CPR/BLS) certification
• MSN degree from an accredited program
• Board certified by AANP or ANCC
• At least 6 months of current Nurse Practitioner clinical experience preferred

This role offers competitive compensation, with $80$90 paid per completed evaluation, plus mileage reimbursement and compensation for administrative time.
