Job Description

Advance your clinical practice with the opportunity to perform impactful work as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage members in Harnett County, NC. This per diem position is ideal for an experienced NP seeking autonomy, flexibility, and the chance to provide high-quality care directly in patients' homes.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Build your own schedule to fit your availability
  • Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

  • Conduct comprehensive annual health risk assessments in patient homes
  • Review and evaluate prior/current medical history, chronic conditions, and medication regimens
  • Deliver patient education regarding conditions and prescribed medications
  • Identify gaps in care and offer recommendations for follow-up or intervention
  • Visits will vary, including post-discharge and annual assessment appointments; plan for approximately 45 minutes per visit
  • No prescribing or initiation of treatment plans required

Desired Qualifications:

  • Active and unencumbered Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • National Board Certification (AANP or ANCC)
  • MSN from an accredited nursing program
  • At least 6 months recent NP experience preferred

You will serve Medicare Advantage members across Harnett County, including areas such as Dunn, Lillington, Anderson Creek, and Spout Springs. Youll be scheduled in one or more of these locations per shift, providing essential health screenings and education.

Successful applicants will receive competitive pay of $80$90 per evaluation, along with a mileage stipend and compensation for administrative time.
