Seeking a highly skilled Nurse Practitioner to perform comprehensive home health risk assessments across Washington Parish, Louisiana. This unique per diem opportunity allows you to make a meaningful impact by visiting Medicare Advantage plan members in their homeshelping to bridge gaps in care and support positive health outcomes.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule to accommodate personal and professional needs
• Commit to a minimum number of hours on a weekly or bi-weekly basis

Role Overview:

• Conduct in-home risk assessments for Medicare Advantage members, including annual health evaluations and post-discharge follow-ups
• Review and evaluate prior and current medical history, chronic conditions, and medication use
• Provide patient education about diagnoses, preventive care, and medication management
• Identify gaps in care and offer clinical recommendations without developing treatment plans or prescribing medications
• Visits are scheduled throughout Bogalusa, Franklinton, Angie, and surrounding areas within Washington Parish

Key Qualifications:

• Unencumbered Nurse Practitioner license in Louisiana
• Active Board Certification (AANP or ANCC)
• Valid BLS (CPR) certification
• Minimum of six months experience as a Nurse Practitioner preferred
• MSN degree from an accredited nursing program
