Deliver comprehensive healthcare in a flexible per diem role conducting home health risk assessments as a Nurse Practitioner. This opportunity involves visiting Medicare Advantage plan members in their homes throughout Taos County, providing a valuable service by assessing their health needs and helping to bridge gaps in care. Locations may include Taos, Questa, Red River, and other nearby areas. Every assessment is designed to last approximately 45 minutes, focusing on annual health reviews and occasional post-discharge visits.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Design your own schedule to fit your availability
• Minimum commitment required on a weekly or bi-weekly basis

Responsibilities:

• Conduct annual risk assessments for Medicare Advantage members in their homes
• Review patients medical histories, conditions, and medication lists in detail
• Educate patients about their conditions and prescribed medications
• Detect gaps in care and offer actionable recommendations
• No responsibilities for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in New Mexico
• CPR/BLS certification
• Board certification from AANP or ANCC
• Six months or more of recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution

Compensation:

• Earn $80$90 per evaluation, plus receive a mileage stipend and payment for administrative time
