Deliver high-quality, patient-centered care as a Nurse Practitioner specializing in home health assessments. In this per diem role, youll have the flexibility to create your own schedule and make a significant impact on Medicare Advantage plan members living throughout Campbell County, including areas like LaFollette, Jacksboro, and Jellico. Each risk assessment visit averages 45 minutes, with assignment locations varying by shift.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling options to fit your availability
- Minimum weekly or bi-weekly hour commitment required
Key Responsibilities:
- Perform comprehensive annual home health risk assessments on Medicare Advantage members
- Review and evaluate prior and current medical histories, ongoing conditions, and medication compliance
- Educate patients regarding their medical conditions and medications to promote understanding and adherence
- Identify gaps in care, providing actionable recommendations for improvement
- No ongoing treatment planning or prescribing required during assessments
Qualifications:
- Valid Nurse Practitioner License in TN
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- At least six months of Nurse Practitioner experience preferred
Enjoy compensation between $80 and $90 per evaluation, with additional mileage reimbursement and stipend for administrative time.
