Advance your nurse practitioner career by delivering high-quality care in a per diem home health role. This opportunity offers flexibility and the chance to make a meaningful impact by performing health risk assessments for Medicare Advantage plan members across Campbell County, TN, including LaFollette, Jacksboro, and Jellico. The position involves traveling to patient residences and providing essential health evaluations tailored to each individual's needs.

Schedule:

• Flexible shifts offered Monday through Sunday
• Design your own schedule to fit your availability
• Option to commit to minimum weekly or bi-weekly hours

Core Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage members
• Review and evaluate previous and current medical histories, as well as medication usage
• Educate patients about ongoing medical conditions and medication management
• Detect care gaps and recommend solutions to ensure optimal health outcomes
• Note: There is no requirement to create treatment plans or prescribe medications

Desired Qualifications:

• Active Nurse Practitioner License (TN)
• Board Certification with AANP or ANCC
• MSN from an accredited nursing program
• Current BLS/CPR certification
• At least 6 months of recent nurse practitioner experience preferred
