Advance patient care in the community by performing in-home health risk assessments. As a Nurse Practitioner specializing in home health, you will connect with Medicare Advantage plan members throughout Delaware Countyincluding areas such as Muncie, Yorktown, and Chesterfieldto evaluate health needs and promote wellness. This per diem role offers a highly flexible schedule where you can select your own working days and hours to suit your availability.
Schedule:
- Flexible shifts available Monday through Sunday
- Option to create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review patients prior and current medical histories, chronic conditions, and medication use
- Deliver clear education about medical diagnoses and medications
- Identify gaps in care and provide actionable recommendations for improved health outcomes
- No treatment plan development or medication prescribing involved
Qualifications:
- Active Nurse Practitioner license in Indiana
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Current CPR (BLS) certification
- Minimum of 6 months NP experience preferred
