Job Description
Deliver valuable healthcare directly to patients by performing home health risk assessments for Medicare Advantage plan members in Delaware County, Indiana. This per diem position empowers you to set your own schedule with flexible shifts available any day of the week. Each home visit typically takes around 45 minutes, with assessments varying from annual health checks to post-discharge evaluations. Locations may include Center Township and surrounding areas such as Muncie, Yorktown, and Chesterfield, with the opportunity to be scheduled in any of these during your shifts.
Schedule:
- Flexible shifts between Monday and Sunday
- Self-scheduling to fit your personal and professional needs
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical history, chronic medical conditions, and medication usage
- Provide patient education focused on health conditions and prescribed medications
- Identify care gaps and recommend solutions as needed
- No development of treatment plans or prescribing medications is involved in this role
Qualifications:
- Active Nurse Practitioner license in Indiana
- Board Certification via AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- Six months or more of NP experience preferred
Nurse Practitioners in this dynamic role receive between $80 and $90 for each evaluation, along with additional compensation for mileage and administrative time.
