Job Description

Advance your career as a Nurse Practitioner by providing essential home health risk assessments for Medicare Advantage plan members across Fulton County, NY. This per diem opportunity offers the flexibility to create your own schedule with shifts available throughout the week, including weekends. Enjoy a dynamic practice environment where each assessment visit spans approximately 45 minutes, focusing on comprehensive annual health evaluations or post-discharge check-ins at patients residences throughout locations such as Gloversville, Johnstown, and Broadalbin.

Schedule:

  • Flexible shifts available Monday through Sunday
  • You have the opportunity to build your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Primary Responsibilities:

  • Conduct thorough home health risk assessments for Medicare Advantage members
  • Review and evaluate patients prior and current medical and medication histories
  • Educate patients about their health conditions and medication regimens
  • Identify gaps in care and make actionable recommendations
  • Focus exclusively on assessmentsno treatment planning or medication prescribing involved

Desired Qualifications:

  • Active Nurse Practitioner license in New York State
  • Current CPR certification (BLS)
  • Board certification as an NP (AANP or ANCC)
  • Minimum of 6 months recent NP experience preferred
  • Master of Science in Nursing from an accredited institution

This position offers compensation of $80-$90 per evaluation, along with a mileage stipend and added reimbursement for administrative time.
