Job Description

Make a meaningful impact in community health by conducting dedicated home health risk assessments for Medicare Advantage members across Fulton County, NY, including areas such as Gloversville, Johnstown, and Broadalbin. This per diem opportunity offers autonomy and flexibility for Nurse Practitioners seeking to provide high-quality care while enjoying a self-directed schedule.

Schedule/Hours:

  • Flexible shifts available any day between Monday and Sunday
  • Set your own schedule to fit your availability
  • Choose between weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Complete annual home health risk assessments in members homes, averaging about 45 minutes per visit
  • Review and evaluate both prior and current medical histories, medical conditions, and medication usage
  • Educate patients regarding their health conditions and medication management
  • Identify care gaps and offer professional recommendations to help close those gaps
  • No development of ongoing treatment plans or medication prescribing required

Qualifications:

  • Current and unrestricted Nurse Practitioner license in New York State
  • Active CPR certification (BLS)
  • Board Certified through AANP or ANCC
  • At least 6 months of recent Nurse Practitioner experience preferred
  • Masters degree in Nursing (MSN) from an accredited institution
