Job Description

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate prior/current medical history, chronic conditions, and medication regimens
  • Educate patients and their families on health conditions, disease prevention, and medication usage
  • Identify care gaps, and offer recommendations to support care coordination and improved health outcomes
  • No treatment planning or medication prescribing involvedfocus exclusively on assessment and education

Qualifications:

  • Active Nurse Practitioner license in Virginia
  • Current BLS/CPR certification
  • Board Certification through AANP or ANCC
  • MSN from an accredited nursing program
  • At least 6 months of current Nurse Practitioner experience preferred

You'll deliver care through home visits, with each comprehensive risk assessment lasting approximately 45 minutes. The types of visits will vary, including annual health assessments and post-discharge follow-ups, all conducted in the homes of Medicare Advantage members.
