Deliver vital healthcare services in the community by performing home health risk assessments as a Nurse Practitioner. This per diem role offers the flexibility to create your own schedule with shifts available seven days a week, ensuring a balanced work-life routine. Practitioners will visit Medicare Advantage plan members across Campbell County, including Timberlake, Altavista, and Brookneal, providing essential assessments that contribute to quality patient care.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling to fit your availability
• Set minimum weekly or bi-weekly hour commitments

Responsibilities:

• Conduct comprehensive annual health risk assessments in patients' homes
• Review and evaluate prior and current medical history, medical conditions, and medication histories
• Provide patient education regarding health conditions, medications, and preventive care
• Identify care gaps, offering recommendations to close those gaps
• Complete thorough documentationno need to develop treatment plans or prescribe medications

Qualifications:

• Nurse Practitioner license valid in Virginia
• Current CPR certification (BLS)
• Certification by AANP or ANCC
• Minimum of 6 months recent NP experience (preferred)
• MSN degree from an accredited nursing program
