Shape the future of patient care as an advanced practice provider conducting home health risk assessments in Power County, ID. This per diem opportunity offers you the flexibility to design your schedule and focus on meaningful patient interactions in various locations, including American Falls and Rockland.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hour commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Evaluate patients prior and current medical history, conditions, and medication usage
- Offer education and guidance regarding medical diagnoses and medications
- Detect care gaps and recommend solutions for optimal patient outcomes
- Assessments may include annual evaluations or post-discharge visits
- No responsibility for creating treatment plans or prescribing medicationsfocus remains on holistic assessment and education
Qualifications:
- Active Nurse Practitioner license in Idaho
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree earned from an accredited nursing program
- At least six months of direct NP experience preferred
