Advance your career as a Nurse Practitioner by conducting home health risk assessments throughout Whiteside County, Illinois. This per diem position focuses on providing high-quality evaluations for Medicare Advantage plan members in communities such as Sterling, Rock Falls, and Morrison. Each assessment is designed to take approximately 45 minutes, offering flexibility and varietyranging from annual health assessments to post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own working days and times
- Minimum commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients' prior and current medical histories, conditions, and medication use
- Offer education to patients regarding medical conditions and medications
- Identify gaps in care and propose actionable recommendations
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Illinois
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of current Nurse Practitioner experience preferred
