Home health risk assessment professionals dedicated to making a difference are invited to perform essential services for Medicare Advantage plan members in Amarillo, Bushland, Panhandle, Fritch, and communities throughout Potter County, TX. This per diem opportunity is tailored for Nurse Practitioners seeking a flexible role with a valuable purpose in patients homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hour commitments
Key Responsibilities:
- Complete thorough annual or post-discharge home health risk assessments (each lasting about 45 minutes) with Medicare Advantage members
- Review and evaluate current and prior medical conditions, histories, and medications
- Educate members regarding their diagnoses, treatments, and prescribed medications
- Identify care gaps and offer appropriate recommendations for additional care or resources
- No responsibilities for writing prescriptions or treatment plans
Qualifications:
- Active Nurse Practitioner license in Texas
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of experience as a Nurse Practitioner preferred
