Schedule:

  • Flexible shifts available any day between Monday and Sunday

Key Responsibilities:

  • Perform comprehensive annual risk assessments for Medicare Advantage health plan members in their homes
  • Review patients prior and current medical histories, health conditions, and medication regimens
  • Educate members on their medical conditions and prescribed therapies
  • Identify care gaps and make recommendations to improve overall health outcomes
  • Note: No need for treatment plan creation or medication prescribing

Preferred Qualifications:

  • Active Nurse Practitioner license in Florida
  • Current CPR certification (BLS)
  • Board certification via AANP or ANCC
  • Minimum of six months of experience working as an NP is preferred
  • Graduate of an accredited MSN program

In this position, youll earn $80$90 per completed home health evaluation, with additional mileage stipend and compensation for administrative time. Home health visits last approximately 45 minutes, including both annual assessments and post-discharge follow-up, allowing you to utilize your clinical expertise in a supportive, educational context.
