Job Description
Deliver high-quality care and support within the comfort of patients homes as a Nurse Practitioner conducting comprehensive home health risk assessments throughout Calhoun County. This per diem role is ideal for a skilled clinician seeking autonomy, flexibility, and meaningful patient interaction.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling options
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review and evaluate patient medical history, current conditions, and medication usage
- Educate patients on their medical diagnoses and prescribed medications
- Identify gaps in care and provide informed recommendations for ongoing management
- No responsibility for creating treatment plans or prescribing medication
Qualifications:
- Active Nurse Practitioner license in Florida
- Current CPR certification (BLS)
- Valid Board Certification (AANP or ANCC)
- Minimum of 6 months recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited program
In this role, the average assessment will take approximately 45 minutes. Assignments may include annual health assessments or post-discharge visits, and may be scheduled in Altha, Blountstown, or other parts of Calhoun County. Mileage stipends and compensation for administrative time are provided, in addition to competitive per-evaluation earnings of $80-$90.
