Experienced Nurse Practitioners with a passion for home health are needed to conduct thorough in-home risk assessments for Medicare Advantage plan members across Volusia County. In this per diem opportunity, you will independently visit members in their residences, performing detailed evaluations aimed at improving health outcomes and ensuring care plan accuracy.
Schedule:
- Flexible shifts available seven days a week (Monday through Sunday)

- Choose your minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments
- Review and document prior and current medical history, conditions, and medications
- Educate members on health conditions and medication management
- Identify gaps in care and provide recommendations for improvement
- Each evaluation averages 45 minutes, ranging from annual assessments to post-discharge visits
- No treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner License in Florida (required)
- Current Basic Life Support (BLS) CPR certification
- Board Certification via AANP or ANCC
- Minimum of six months recent experience as an NP (preferred)
- MSN degree from an accredited nursing program
