Enjoy a dynamic, independent role delivering in-home healthcare assessments to Medicare Advantage plan members across Crawford County. As a Nurse Practitioner specializing in home health, you will support patient wellness through comprehensive risk assessmentsensuring community members receive the quality care and follow-up they need.
Schedule:
- Flexible shifts available Monday through Sunday
- Craft your own schedule
- Commit to a minimum number of weekly or bi-weekly hours for consistent engagement
Responsibilities:
- Complete annual and post-discharge health risk assessments in patient homes (approximately 45 minutes per visit)
- Review prior and current medical histories, chronic and acute conditions, and medication usage
- Provide patients with education regarding health conditions and proper medication use
- Pinpoint gaps in care, offering thoughtful recommendations to bridge them
- No prescribing or development of long-term treatment plans required
Desired Qualifications:
- Active Nurse Practitioner license in Arkansas
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- MSN from an accredited nursing program
- Six or more months of recent nurse practitioner experience preferred
As part of this role, youll receive $80$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time. Youll have autonomy over your work schedule, allowing you to achieve balance and flexibility while making a direct impact on the health of residents in Alma, Van Buren, Mulberry, and surrounding communities.
