This per diem opportunity is suited for nurse practitioners skilled in home health, with responsibilities centered on conducting risk assessments for Medicare Advantage plan members throughout Crawford County. Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review medical histories, conditions, and medication regimens
- Educate patients on their health conditions and therapies
- Identify gaps in care and make recommendations for improved outcomes
- No requirement to create treatment plans or prescribe medications
Desired Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current BLS (CPR) certification
- Board certified through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
You'll conduct assessments in patients homes throughout Van Buren, Alma, Mulberry, and other locations in Crawford County. Most assessments take around 45 minutes.
This position offers a competitive per-assessment rate of $80-$90, along with a mileage stipend and compensation for administrative time.
