Step into a rewarding per diem opportunity performing home health risk assessments for Medicare Advantage plan members throughout Columbiana County, OH. This Nurse Practitioner position offers flexible scheduling and a chance to positively impact patients lives by providing crucial assessments and education in their own homes.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Option to create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual home health risk assessments, each lasting approximately 45 minutes
- Travel to various locations within Columbiana County, including Perry Township, Salem, and East Liverpool
- Review and document comprehensive medical history, medical conditions, and medication use
- Educate patients about their medical conditions and medications, helping them make informed health decisions
- Identify gaps in care and offer actionable recommendations for ongoing patient wellness
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- At least 6 months of NP experience is preferred
- MSN from an accredited nursing program
This role offers compensation of $80$90 per evaluation, along with a mileage stipend and payment for administrative time, providing both professional satisfaction and financial reward.
