Take on a rewarding opportunity as a Nurse Practitioner delivering home health risk assessments for Medicare Advantage plan members residing in Pontotoc County, OK, including Ada, Allen, and Stonewall. This per diem role empowers you to create your own flexible schedule and serve a diverse community by completing important health evaluations in patient homes. Each risk assessment lasts approximately 45 minutes and may include annual evaluations or follow-up visits after hospital discharge.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-schedulingdesign shifts that match your availability
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Review and document prior and current medical history, medical conditions, and medications
• Educate patients on diagnoses, medication use, and ongoing care needs
• Identify any gaps in care and offer recommendations for improved health outcomes
• No requirements for treatment planning or medication prescribing

Qualifications:

• Active Nurse Practitioner license in Oklahoma
• Master's of Science in Nursing (MSN) from an accredited institution
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• At least six months of nurse practitioner experience preferred
