Job Description

Deliver essential care directly to patients' homes as a Nurse Practitioner performing comprehensive home health risk assessments. This per diem position offers flexibility in scheduling and an impactful role supporting Medicare Advantage members in Kalamazoo County, MI, covering areas such as Kalamazoo, Portage, and Oshtemo.

Schedule:

  • Flexible shifts available Monday through Sunday
  • The freedom to create your own schedule
  • Minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Complete annual risk assessments in patient homes for Medicare Advantage health plan members
  • Review and document prior/current medical histories, active conditions, and medication usage
  • Offer patient education about medical conditions and medication management
  • Identify and communicate any gaps in care, providing recommendations for improvement
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Michigan
  • Board Certification (AANP or ANCC)
  • Current BLS CPR certification
  • MSN degree from an accredited nursing program required
  • At least 6 months of current Nurse Practitioner experience preferred
