Enhance patient care in the community by providing expert in-home health assessments as a per diem Nurse Practitioner. This opportunity involves conducting home health risk assessments for Medicare Advantage plan members residing in various locations within Allegany County, including Alfred, Cuba, Caneadea, and Wellsville. Visits may include annual health assessments or post-discharge evaluations, each lasting approximately 45 minutes. Scheduling is highly flexible, allowing you to design shifts between Monday and Sunday to fit your availability, with minimum weekly or bi-weekly commitments.

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Assess prior and current medical history, medical conditions, and medication usage
• Deliver patient education regarding medical conditions and prescribed medications
• Identify gaps in care and suggest evidence-based recommendations for improvement
• Note: No prescribing or development of treatment plans required

Qualifications:

• Active Nurse Practitioner license in New York
• Current BLS/CPR certification
• Board certification via AANP or ANCC
• Minimum of 6 months recent NP experience (preferred)
• MSN from an accredited nursing program
