Key Responsibilities:

• Conduct annual and post-discharge health risk assessments in patients homes
• Review and evaluate patients' medical history, current conditions, and medication use
• Provide clear education regarding disease management and medication protocols
• Identify and communicate any care gaps, offering actionable recommendations
• No treatment plan development or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in the state of New York
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of current Nurse Practitioner experience preferred
