Delivering meaningful healthcare to patients in their own homes, this per diem Nurse Practitioner opportunity focuses on performing home health risk assessments throughout Allegany County, including communities such as Alfred, Cuba, and Caneadea. Schedule / Hours

  • Flexible shifts available Monday through Sunday
  • Self-selected scheduling

Primary Responsibilities

  • Conduct comprehensive home health risk assessments averaging 45 minutes per visit
  • Assess prior and current medical history, ongoing conditions, and medications
  • Educate patients on disease management, prevention, and medication adherence
  • Identify gaps in care and suggest practical recommendations for improvement
  • Visits may include annual assessments and post-discharge evaluations
  • No requirements for formulating treatment plans or prescribing medications

Qualifications

  • Active Nurse Practitioner license in New York State
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • Minimum of six months current NP experience preferred
  • MSN from an accredited nursing program
