Deliver high-quality patient care and make a meaningful impact by providing in-home risk assessments for Medicare Advantage members. This per diem Nurse Practitioner opportunity offers the flexibility to manage your own schedule while working within Delaware County, IN. Candidates may conduct visits in areas such as Muncie, Yorktown, and Chesterfield, with each risk assessment typically lasting around 45 minutes. The position primarily involves annual health assessments or post-discharge visits, focusing on patient education and care gap identification rather than ongoing management or prescribing.

Schedule:

• Flexible shifts available between Monday and Sunday
• Ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for a Medicare Advantage health plan
• Review medical history, current conditions, and medication regimens
• Educate patients regarding their health conditions and prescribed medications
• Identify gaps in care and recommend actions to address them
• No requirement to create treatment plans or prescribe medication

Qualifications:

• Active Nurse Practitioner license in Indiana
• Current CPR certification (BLS) required
• Board Certification through AANP or ANCC
• Minimum of 6 months of recent Nurse Practitioner experience is preferred
• MSN degree from an accredited institution
