Job Description

Deliver meaningful care directly to patients' homes as a Nurse Practitioner performing home health risk assessments for Medicare Advantage plan members. This per diem position offers flexibility and the chance to design your own schedule, with opportunities for shifts between Monday and Sunday and a minimum weekly or bi-weekly hourly commitment to fit your lifestyle. Candidates can expect to serve a diverse population throughout Delaware County, including areas such as Muncie, Yorktown, Chesterfield, and Center Township.

Responsibilities:

  • Conduct comprehensive annual home health risk assessments, averaging 45 minutes per visit
  • Review patients prior and current medical history, medical conditions, and medication usage
  • Educate patients on their health conditions and prescribed medications
  • Identify care gaps and provide clear recommendations to improve patient outcomes
  • Perform post-discharge and annual assessment visits as needed
  • No treatment planning or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Board certification from AANP or ANCC
  • Current CPR certification (BLS)
  • Master of Science in Nursing from an accredited institution
  • At least 6 months of recent Nurse Practitioner experience preferred
