Step into a vital role as a Nurse Practitioner specializing in home health, providing comprehensive risk assessments for Medicare Advantage plan members in Wyoming County, NYincluding towns like Attica, Warsaw, and Perry. This per diem opportunity offers remarkable flexibility, allowing you to shape your own schedule and set a minimum weekly or bi-weekly hourly commitment.
Schedule:
- Flexible shifts available any day from Monday through Sunday
- Self-scheduling to meet your personal needs
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members in their homes
- Review prior and current medical history, existing conditions, and medication usage
- Educate patients on their conditions and prescribed medications to enhance understanding and compliance
- Identify gaps in care, offering recommendations to address and resolve these areas
- No responsibilities for developing treatment plans or prescribing medications
Qualifications:
- Current NY State Nurse Practitioner license
- Active CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN from an accredited nursing program
- Minimum of 6 months of recent experience as a Nurse Practitioner preferred
