Schedule:

  • Flexible shifts available any day from Monday through Sunday
  • Self-scheduling to meet your personal needs
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage members in their homes
  • Review prior and current medical history, existing conditions, and medication usage
  • Educate patients on their conditions and prescribed medications to enhance understanding and compliance
  • Identify gaps in care, offering recommendations to address and resolve these areas
  • No responsibilities for developing treatment plans or prescribing medications

Qualifications:

  • Current NY State Nurse Practitioner license
  • Active CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN from an accredited nursing program
  • Minimum of 6 months of recent experience as a Nurse Practitioner preferred
