This per diem opportunity allows you to deliver care in the community, supporting members throughout Albuquerque and the wider Bernalillo County region. Schedule:
- Flexible shifts available Monday through Sunday
- Build your own schedule for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform thorough annual risk assessments for Medicare Advantage plan participants in home settings
- Review and evaluate comprehensive medical, medication, and health histories
- Offer education on health conditions and proper medication usage
- Identify care gaps and provide actionable recommendations for follow-up, ensuring continuity of care
- Assessments do not involve direct treatment planning or medication prescribing
Qualifications:
- Active Nurse Practitioner license in New Mexico
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree awarded by an accredited school of nursing
- At least six months of experience as a Nurse Practitioner preferred
You will be compensated between $80 and $90 per evaluation, with an additional mileage stipend and compensation for administrative time.
