Take your Nurse Practitioner experience beyond the clinic setting by providing high-impact care directly in patient homes. This per diem opportunity focuses on conducting thorough risk assessments for Medicare Advantage plan members within Marion County, including areas such as Salem, Keizer, Woodburn, and Hayesville.

Key Responsibilities:
- Complete comprehensive annual home health risk assessments for health plan members
- Review and evaluate prior and current medical history, medical conditions, and medication history
- Educate patients on their conditions and medications to empower better outcomes
- Identify potential gaps in care, recommending necessary next steps to optimize health
- Focus strictly on evaluationno need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Oregon
- Current CPR (BLS) certification
- Board certification through AANP or ANCC
- MSN from an accredited nursing school
- Minimum of 6 months current NP experience preferred
