Job Description

Deliver vital care in a dynamic home health setting as a Nurse Practitioner, conducting risk assessments for Medicare Advantage plan members throughout Meigs County, Ohioincluding locations such as Middleport, Pomeroy, Syracuse, and Racine. This per diem opportunity is perfect for experienced practitioners who value flexibility while making a direct impact on patient wellness.

Schedule:

  • Flexible shifts available seven days a week
  • Ability to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment, as agreed upon

Key Responsibilities:

  • Perform in-home annual risk assessments and post-discharge visits, each averaging approximately 45 minutes
  • Review and evaluate medical history, current conditions, and medications
  • Educate patients on health conditions, prescribed treatments, and medication adherence
  • Identify care gaps and provide informed recommendations for care improvement
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Valid Nurse Practitioner license in Ohio
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • Current BLS (CPR) certification
  • At least six months of recent NP experience preferred

This position provides compensation of $80 to $90 per evaluation, along with a mileage stipend and compensation for administrative time.
