As a Nurse Practitioner specializing in home health, you'll play an integral part in improving patient outcomes for Medicare Advantage members. Locations may include Middleport, Pomeroy, Syracuse, Racine, and surrounding communities.
Schedule:
- Flexible shifts available Monday through Sunday

- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments (approximately 45 minutes each)
- Review and update patients current and prior medical histories, including conditions and medication lists
- Educate patients on their health conditions and medication regimens
- Identify care gaps, making recommendations to optimize patient well-being
- Focus assessments only; no diagnosis, treatment plan creation, or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- Minimum of 6 months recent NP experience is preferred
- MSN from an accredited nursing program
