As a Nurse Practitioner specializing in home health, you'll play an integral part in improving patient outcomes for Medicare Advantage members. Locations may include Middleport, Pomeroy, Syracuse, Racine, and surrounding communities.

Schedule:

  • Flexible shifts available Monday through Sunday

  • Choose minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Complete annual and post-discharge home health risk assessments (approximately 45 minutes each)
  • Review and update patients current and prior medical histories, including conditions and medication lists
  • Educate patients on their health conditions and medication regimens
  • Identify care gaps, making recommendations to optimize patient well-being
  • Focus assessments only; no diagnosis, treatment plan creation, or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • Minimum of 6 months recent NP experience is preferred
  • MSN from an accredited nursing program
