Home health nurse practitioners play a vital role in improving patient outcomes through direct, in-home risk assessments. This per diem opportunity offers exceptional flexibility, allowing you to design your own schedule with shifts available any day of the week. Assignments are located throughout Albany County, including areas such as Cohoes, Watervliet, and Westmere.

Schedule Details:

• Flexible shifts offered Monday through Sunday
• Create your own schedule to fit your lifestyle
• Choose minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, conditions, and medication usage
• Educate patients about their health conditions and medications
• Identify care gaps and provide recommendations to support patient well-being
• Visits may include annual assessments or post-discharge check-ins
• No responsibility for treatment plan creation or medication prescribing

Qualifications:

• Active Nurse Practitioner license in the state of New York
• MSN degree from an accredited nursing school
• Current CPR (BLS) certification required
• Board certification through AANP or ANCC
• At least 6 months of current NP experience preferred

Compensation & Benefits: This position offers $80$90 per evaluation, with additional mileage reimbursement and compensation for administrative time.
