Take on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. In this role, you will travel within Schenectady County, NYincluding Schenectady, Rotterdam, and Glenvilleto evaluate Medicare Advantage plan members in their homes. Your work will support comprehensive, quality care and contribute meaningfully to preventative health.
Schedule:
- Flexible shifts, Monday through Sunday
- Create your own schedule
- Commitment to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review prior and current medical histories, medical conditions, and medication histories
- Educate patients on medical conditions and medication usage
- Identify gaps in care and recommend follow-up actions to address potential issues
- No treatment plans or prescribing responsibilities required
Qualifications:
- Active Nurse Practitioner license in New York
- Current CPR certification (BLS)
- Board certification via AANP or ANCC
- Masters degree in nursing (MSN) from an accredited program
- At least 6 months recent NP experience preferred
