Job Description

Deliver comprehensive care in the community by performing home health risk assessments as a Nurse Practitioner (NP). This per diem position offers the unique opportunity to evaluate Medicare Advantage members in various locations within Schenectady County, NY, including Schenectady, Rotterdam, and Glenville. Each assessment averages approximately 45 minutes, focusing on either annual health reviews or post-discharge evaluations.

Enjoy the flexibility of creating your own schedule, with shifts available any day between Monday and Sunday. Commitment can be tailored to your availability, with minimum weekly or bi-weekly hour requirements.

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical conditions, histories, and medications
  • Educate members regarding their medical conditions and medication management
  • Identify gaps in care and recommend appropriate next steps
  • No prescribing or treatment plan development is necessary

Qualifications:

  • Active Nurse Practitioner license in the state of NY
  • Board certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited nursing school
  • Minimum of 6 months current NP experience preferred

This role offers compensation of $80$90 per evaluation, including a mileage stipend and administrative time.
