Job Description

Deliver meaningful patient care as a Nurse Practitioner in a dynamic home health setting throughout Rensselaer County. As part of this flexible per diem opportunity, you will conduct home health risk assessments for Medicare Advantage plan members in areas such as Troy, Rensselaer, and Hoosick Falls. The focus is on comprehensive annual health evaluations and post-discharge visits, each typically lasting about 45 minutes. No on-call, treatment planning, or medication prescribing is required.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling set your own hours
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform thorough home health risk assessments as part of a Medicare Advantage program
  • Review and evaluate patients prior and current medical history, conditions, and medications
  • Educate members about their health conditions and prescribed treatments
  • Spot any gaps in care and deliver actionable recommendations to help close those gaps
  • Operate strictly in an assessment and educational capacity, with no prescribing or treatment planning required

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Current Basic Life Support (BLS) certification
  • Board certification via AANP or ANCC
  • Minimum of 6 months recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
