Job Description

Elevate your clinical expertise in a rewarding per diem opportunity as a Nurse Practitioner specializing in Home Health risk assessments. In this role, you will visit Medicare Advantage plan members at their residences throughout Albany Countyincluding Cohoes, Watervliet, Westmere, and nearby communitiesto perform comprehensive evaluations. Each assessment typically lasts about 45 minutes, and you may be completing either an annual evaluation or a post-discharge visit, depending on the members needs.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule to fit your lifestyle
  • Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

  • Perform annual health risk assessments for Medicare Advantage members in their homes
  • Review and evaluate both current and previous medical history, ongoing conditions, and medication usage
  • Educate patients about their medical conditions and prescriptions to promote optimal health outcomes
  • Identify care gaps and make recommendations to address them
  • No responsibility for creating treatment plans or prescribing medication

Qualifications:

  • Active Nurse Practitioner license in the state of New York
  • Current Basic Life Support (BLS) certification
  • Board certification through AANP or ANCC
  • Graduate of an accredited MSN program
  • At least 6 months of recent Nurse Practitioner experience preferred
