Make a meaningful impact as a Nurse Practitioner specializing in home health assessments by providing critical care services to Medicare Advantage members across Alamosa County. This per diem opportunity offers unmatched flexibility, allowing you to set your own hours and work locations within communities such as Alamosa, Blanca, and Hooper.

Schedule:

• Flexible shifts offered Monday through Sunday
• Possibility to create your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for plan members.
• Review and evaluate medical history, active conditions, and medication regimens.
• Educate patients on their health conditions and medication usage.
• Identify gaps in care and offer recommendations to support patient outcomes.
• Note: Developing treatment plans or prescribing medications is not part of this role.

Qualifications:

• Active Nurse Practitioner licensure in Colorado.
• Board Certification through AANP or ANCC required.
• MSN degree from an accredited nursing program.
• Current CPR (BLS) certification.
• Minimum of six months recent NP experience preferred.
