Job Description

Take on a dynamic per diem opportunity to deliver high-quality care through home health risk assessments in Dyer County, Tennessee. Schedule:

  • Flexible shifts available seven days a week
  • Self-scheduling, allowing you to design your own workweek
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan members
  • Assess patient histories, evaluate current medical conditions, and review medication usage
  • Provide clear patient education regarding their health conditions and medication management
  • Identify gaps in care and offer informed recommendations for improved outcomes
  • Visits may include annual health assessments and post-discharge visits; each assessment typically takes 45 minutes
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Master of Science in Nursing (MSN) from an accredited program
  • Board Certification via AANP or ANCC
  • Current BLS (CPR) certification
  • At least 6 months of recent Nurse Practitioner experience preferred

For each completed evaluation, youll receive competitive compensation of $8090, plus mileage stipend and administrative time allowances.
