Deliver vital care in a unique setting by conducting home health risk assessments for Medicare Advantage members throughout Dyer County. This per diem opportunity is tailored for Nurse Practitioners who are passionate about providing personalized care directly in patients homes.
Schedule:
- Flexible shifts available any day between Monday and Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients' medical histories, existing conditions, and medication regimens
- Deliver clear education to patients regarding their health conditions and prescribed medications
- Identify gaps in care, offering recommendations to promote better health outcomes
- Conduct annual health evaluations or post-discharge visits as assigned
- No requirement for treatment planning or prescription writing
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS)
- Board Certified through AANP or ANCC
- Minimum of 6 months recent experience as a Nurse Practitioner preferred
- MSN from an accredited nursing school
This role offers compensation of $80-$90 per evaluation, along with a mileage stipend and payment for administrative time.
