Advance your career as a Nurse Practitioner specializing in home health risk assessments with a per diem opportunity serving Medicare Advantage members throughout Alachua County. You'll engage directly with patients in their homes, delivering comprehensive risk assessments that contribute to their continued care and well-being.
Schedule:
- Flexible shifts available seven days a week
- Set your own schedule tailored to your availability
- Minimal weekly or bi-weekly hourly commitments, offering excellent work-life balance
Key Responsibilities:
- Conduct in-depth, annual home health risk assessments for Medicare Advantage members; each assessment typically lasting about 45 minutes
- Review patients prior and current medical history, medical conditions, and medication regimens
- Provide valuable education on health conditions and medication adherence
- Identify care gaps and make recommendations for follow-up, ensuring quality healthcare continuity
- No responsibility for treatment plan creation or medication prescribing
Qualifications:
- Valid Nurse Practitioner License in Florida
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of current NP experience preferred
