Job Description

Experienced Nurse Practitioners are needed to perform in-home health risk assessments in the Akron, OH region, serving Medicare Advantage plan members throughout Summit County. This per diem opportunity puts you in control of your schedule, with the flexibility to work in locations including Akron as well as surrounding areas such as Cuyahoga Falls, Stow, and Barberton. Home health risk assessments typically last 45 minutes and may involve annual health evaluations or post-discharge visits.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-schedulingdesign your own work week
  • Minimum weekly or bi-weekly hourly commitment required

Qualifications:

  • Valid Nurse Practitioner license in Ohio
  • Current BLS (CPR) certification
  • Board certified via AANP or ANCC
  • Minimum of 6 months' recent NP experience preferred
  • MSN degree from an accredited nursing program

Key Responsibilities:

  • Conduct comprehensive annual or post-discharge home health risk assessments for Medicare Advantage plan members
  • Review and assess past and present medical history, conditions, and medication usage
  • Offer patient education on disease management and medication safety
  • Identify care gaps and provide actionable recommendations to improve patient outcomes
  • No requirement to create new treatment plans or prescribe medications
