We are seeking an experienced Nurse Practitioner to conduct home health risk assessments for Medicare Advantage plan members in Stark County, Ohio, including Canton, Massillon, North Canton, Alliance, and surrounding areas. This opportunity is ideal for providers passionate about community-based care and comprehensive health evaluations.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule to fit your lifestyle
- Choose your preferred minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Perform annual home health risk assessments in patient homes; each typically takes 45 minutes
- Evaluate medical history, current health conditions, and medication use
- Offer patient education on diagnosed conditions and medications
- Assess and identify gaps in care, providing recommendations for follow-up
- No treatment plan development or prescription duties required
Qualifications:
- Active Nurse Practitioner License in Ohio
- Current CPR certification (BLS)
- Board Certification from AANP or ANCC
- At least 6 months of hands-on NP experience preferred
- MSN degree from an accredited nursing school
