Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling for optimal work-life balance
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct home health risk assessments, each lasting approximately 45 minutes, for Medicare Advantage members in various Portage County locations
  • Evaluate patients current and prior medical history, including medical conditions and medication use
  • Educate patients on health conditions and pharmacological regimens
  • Identify care gaps and recommend appropriate interventions or follow-up actions
  • Visits may involve annual health assessments or post-discharge reviews, depending on member needs

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN from an accredited nursing program
  • At least six months of NP experience is preferred
