An exciting opportunity is available for an experienced Nurse Practitioner to make a difference in patient care through home health risk assessments. This per diem role focuses on evaluating and educating Medicare Advantage plan members residing throughout Hertford County, including communities such as Ahoskie, Murfreesboro, and Winton. Schedule
- Flexible shifts available Monday through Sunday
- Set your own hours and customize your work schedule
- Minimum weekly or bi-weekly hourly commitment
Essential Responsibilities
- Complete annual home health risk assessments in patient homes
- Review and document prior and current medical history, conditions, and medications
- Educate patients on managing medical conditions and medication regimens
- Identify and communicate gaps in care, offering recommendations for follow-up
- Visits typically last about 45 minutes and may include annual assessments or post-discharge check-ins
- No prescribing of medications or creation of treatment plansfocus is on assessment and education
Preferred Qualifications
- Active Nurse Practitioner license in North Carolina
- Board Certification via AANP or ANCC
- Current BLS (CPR) certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least six months of recent nurse practitioner experience preferred
Compensation Earn $80$90 per completed evaluation, along with stipends for mileage and administrative time.
