Job Description

Experience a fulfilling and flexible opportunity as a Nurse Practitioner specializing in home health risk assessments. This per diem role allows you to craft your own schedule with flexible shifts available throughout the week, accommodating both weekday and weekend availability. Candidates will travel throughout Craig County, performing evaluations in areas such as Vinita, Ketchum, Welch, and White Oak.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Self-scheduling with minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct thorough annual home health risk assessments for Medicare Advantage plan members in home settings
  • Review and document prior and current medical history, existing medical conditions, and full medication details
  • Provide clear patient education relating to medical conditions and prescribed medications
  • Identify care gaps and deliver recommendations to bridge those gaps
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Oklahoma
  • Current CPR (BLS) certification
  • Board certification through AANP or ANCC
  • Preferred minimum of 6 months of recent NP experience
  • Masters degree (MSN) from an accredited nursing program
